Specialized Recovery Care for Complex Conditions
Hebrew Rehabilitation Center provides medical and rehabilitative care for patients with complex medical conditions or multiple acute or chronic illnesses through our medically complex care service. This care level is designed for patients who are too ill for rehabilitative care alone, but do not need the full services offered by a major acute care hospital. It can also be appropriate for our long-term chronic care patients should they need a higher level of medical support for complex conditions.
Treatment combines hospital-level care with primary medical care, specialty medical care, and rehabilitation therapies. Since 2004, we have been caring for patients with a wide range of pulmonary, cardiovascular, orthopedic, neurological, and other medical and post-surgical conditions.
Our interdisciplinary team includes physicians, nurse practitioners, nurses, physical, occupational and speech therapists, pharmacists, social workers, dietitians, and chaplains. Physicians representing many medical specialties are available to our on-site clinical staff for consultation.
